Sending application for maintaining the 'Register of Members' on MHI site at free of cost, along with registered address .
  Only application fee Rs.500.00, to be paid, on one time basis.
  Declaration that society/committee of the organization is not dissolved and registration is active in the record of
  GOI or STATE GOV, if it was registered with GOI or STATE GOV.
  & to provide the 'Certificate of Registration' signed by Registar, if it was availed from GOI or STATE GOV.
  Date of the previous election, if it was held.

4   We hereby declare that the society/union/committee/Association/Organization is not dissolved and registration is active in the record of   GOI or STATE GOV, if it was registered with GOI or STATE GOV.
  Not registered and hence not applicable.
5 Did the elections held in the past ?   No.    Yes.    Date of the last election held  
6 Did the society/union/committee/Association/Organization have applied for the registration in any Govt office ?   No. Yes.
  If yes, mention the date of application , with office of     and Application number is  
7 Upload 'Certificate of Registration', if registered
8   We hereby declare that we have read and understood all the terms and conditions and Policies of m/s MHI and we hereby agrees to all the terms and conditions and Policies of m/s MHI.
9   We hereby wish to avail free services of m/s MHI and thereby our above mentioned Society/Committee/Association/Organisation can maintain 'Register of Members' on MHI site.

10 We declare that all the members will provide one of the following Photo I-Card details while filling the membership form.
